Membership Terms

1. Eligibility and enrollment

Loul Dental Studio offers annual dental memberships for adults without dental insurance. Each membership is for one named patient and is valid for eligible services at Loul Dental Studio. Benefits are personal and cannot be transferred.

Our team will confirm which plan fits your care needs before you enroll. If you are new to the practice, we'll explain your first-visit fees before your exam and recommend a plan after evaluating your oral health.

This is a prepaid, in-office dental membership. It is not dental insurance.

2. Annual fees and plan benefits

*Monthly equivalents are shown for comparison only. The annual fee is paid in full at enrollment.

Every plan also includes:

• One digital wellness scan per membership year. This is an optical scan of your teeth that does not use X-rays.
• Routine dental X-rays when clinically indicated, within the following allowance: To be confirmed by the practice.
• One problem-focused emergency evaluation per membership year, with up to two small intraoral X-rays when clinically indicated at that visit.

The emergency evaluation and its X-ray allowance are tracked separately from the dental exam benefit.

Dr. Loul determines when an exam, X-ray, or scan is clinically indicated. Emergency appointments are scheduled based on urgency and availability. Emergency treatment, additional emergency evaluations, and imaging beyond the plan allowance are billed separately. We'll explain any additional fees before care.

3. Eligible treatment discounts

Essential members receive 5% off, Prevention Plus members receive 10% off, and Wellness members receive 15% off the regular practice fee for the following eligible services performed at Loul Dental Studio during an active membership:

• Dental sealants.
• Tooth-colored fillings, inlays, onlays, overlays, natural-tooth crowns, and core buildups.
• To be confirmed by the practice.

If you need active gum disease treatment, such as scaling and root planing, it is billed separately from the periodontal maintenance visits included in Wellness. Any applicable discount will appear on your treatment estimate. We'll review your fees and applicable savings with you before treatment.

4. Payment, term, and renewal

The membership runs for 12 months. Your start and expiration dates are shown at enrollment. The annual fee is due in one payment. Your checkout summary shows the selected plan, any practice-approved credit, and the exact amount charged.

If the practice approves a credit for a first-visit service already paid for, that credit reduces the amount due at enrollment. The credited service counts toward the corresponding benefit for that membership year.

There is no monthly billing or automatic renewal. Your membership expires on the date shown at enrollment. If you choose to renew, we'll disclose the renewal fee and ask for a new payment authorization.

Your payment authorization covers the membership charge shown at checkout. Any additional treatment, imaging, or appointment fees are handled separately.

5. Scheduling and unused benefits

Please schedule your visits within your membership term. Unused benefits expire at the end of the term and do not carry forward.

Our office requires 48 hours' notice to cancel or reschedule an appointment. A $150 late cancellation or missed appointment fee may apply under our office policy. A service that was not performed does not count as used.

6. Cancellation and refunds

1. How to cancel.

You may cancel your membership at any time by calling Loul Dental Studio at (805) 565-5111 or submitting a written request to our office. Cancellation takes effect on the date we receive your request. We'll confirm the cancellation and any refund in writing. Membership benefits and discounts end on the effective cancellation date.

2. Cancellation within 30 days of enrollment.

If you cancel within 30 calendar days of enrollment and have used no membership benefits, we'll refund the full amount you paid for the membership. If you've used benefits, we'll subtract the regular fees for included services you received and the dollar amount of any membership discounts you used. We'll account for any amount you already paid for the same service and provide an itemized calculation. The refund will be no less than $0. Cancellation will not create a new balance for membership benefits already used.

3. Cancellation after 30 days.

The annual membership payment is generally nonrefundable after the first 30 days. Exceptions apply if we end your membership, cannot provide an included service, identify a billing error, or a refund is required by law.

4. Recall visits, reminders, and unused benefits.

We'll send automated reminders by text or email using the phone number or email address on file, based on your communication preferences. Please keep your contact information current. You are responsible for scheduling and completing your recommended recall visits, including regular cleanings or periodontal maintenance, before your membership expires, even if you don't receive a reminder. Please contact us early to allow for appointment availability. Unused benefits expire at the end of your membership term and do not carry forward. We do not issue refunds solely because you did not schedule or use your benefits. The refund and extension provisions in this policy still apply.

5. If Loul Dental Studio ends your membership or cannot provide an included service.

If we end your membership early or discontinue your plan, we'll refund the portion of your membership payment that corresponds to the time remaining in your term. If we cannot provide an included service during your term, we'll offer an extension or an appropriate partial refund. We'll explain your options and any refund calculation in writing.

6. Billing errors and refund timing.

We'll correct any duplicate or incorrect membership charge. If a refund is due, we'll issue it within 15 calendar days of receiving your cancellation request or identifying the reason for the refund. We generally issue refunds to the original payment method. Your bank or card issuer may take additional time to post the credit.

7. Expiration and renewal.

Your membership is paid in one annual payment and expires on the date shown at enrollment. There is no automatic renewal. If you choose to renew, we'll disclose the renewal fee and ask for a new payment authorization.

Any cancellation or refund rights you have under applicable law still apply.

7. Enrollment acknowledgment

Before payment, you'll be asked to confirm:

• I have reviewed and agree to the Membership Terms for my selected plan.
• I am authorized to use the payment method provided and authorize Loul Dental Studio to charge the amount shown at checkout one time.
